Cortisol rhythm changes in perimenopause before almost every other hormonal marker moves visibly.

The HPA axis, the signalling system between your brain and adrenal glands, becomes less responsive. The cortisol curve flattens or dysregulates. The resilience buffer that women carry through chronic stress, long hours, and sustained caregiving begins to thin.

And here is what this looks like from a parts perspective:

The hypervigilance part that kept everything running, it's tired.

Methylation is a biochemical process that runs in virtually every cell in the body, it's involved in DNA repair, neurotransmitter production, hormone detoxification, and immune function.

When methylation is impaired, the impact shows up as brain fog, mood instability, fatigue, poor stress resilience, and treatment-resistant symptoms that don't respond to standard protocols.

MTHFR and COMT are the two genetic variants I look at first.

MTHFR affects the conversion of folate into its active form (5-MTHF). Without active folate, the methylation cycle stalls, and standard folic acid supplementation does not fix it. In some cases, it makes it worse.

COMT affects how quickly the body breaks down estrogen, dopamine, norepinephrine, and epinephrine. A slow COMT variant means these compounds accumulate, which affects mood, sleep, anxiety, and how the body responds to hormonal changes.

When a patient has done everything correctly, the right diet, the right supplements, the right lifestyle, and still can't think clearly, I look here.

The fix is not more of the same stack. It's the right form of the right nutrient, calibrated to the variant.

Zonulin is the protein that controls the tight junctions in your intestinal wall.

When Zonulin is elevated, the barrier loosens. Partially digested food particles, bacterial components, and other molecules cross into circulation — triggering an immune response that manifests everywhere except the gut.

The physiology changed. The advice didn't.

In perimenopause, the cortisol-insulin axis becomes the central variable in body composition, not calories in, not cardio output.

Here's the mechanism:

Caloric restriction and high-intensity cardio both raise cortisol. Elevated cortisol drives insulin resistance. Insulin resistance drives visceral fat accumulation — specifically the midsection weight that won't move no matter how hard she works.

The intervention is not more discipline. It's blood sugar stabilisation and cortisol regulation first.

This means:
— Eating in a way that keeps blood glucose stable (not low-calorie)
— Movement that does not chronically spike cortisol
— Addressing the HPA axis dysregulation that's underneath the metabolic picture

The advice most women were following was written for a different hormonal environment. It wasn't wrong, it was built for a different body.
