👶💦**Did you know that sometimes the most meaningful measure of education is seeing what happens at the very next birth?**💦👶

Earlier this year, a midwife and a doula in Iran enrolled our online Waterbirth Provider Certification Course. After completing the course, they joined Barbara for a personal Zoom Q&A to deepen their knowledge and prepare to bring these principles into their practice.

Then came this message.

“Today we had our first birth at our center after completing your course, and we followed the approach and principles you taught us throughout the birth.”

The mother was encouraged to listen to her body and follow her baby’s movements. As her baby rotated, she instinctively moved and rotated her pelvis in response.

“She did beautifully!... It was such a beautiful experience to watch.”

At 7 cm, she stopped to eat a meal. Without a birth pool yet, she used a warm shower for comfort while continuing to move and follow her body.

After birth, the cord was left undisturbed until it was completely white and the placenta was delivered. Mother and baby remained skin-to-skin as her baby latched and rested peacefully on her chest.

“Thank you for everything you have taught us and for inspiring us to bring this approach to mothers here in Iran.”

👶💦**Did you know that the true impact of Water Birth International's training is seeing that knowledge turned into action?**💦👶

One of the final stops of Barbara Harper ’s extraordinary 109-day summer teaching tour brought her back to Thumbay University Hospital in the UAE, the largest private academic hospital in the Middle East.

Barbara first provided waterbirth training for the hospital’s birth professionals in December 2025. This summer, they invited her back to see what has happened since.

And what a beautiful homecoming it was.

The hospital has remodeled its birth room incorporating Barbara’s recommendations, creating a dedicated environment designed to support safe, gentle, physiologic birth in water.

Even more exciting, the team has already welcomed several babies through waterbirth in their new facilities!

During her visit, Barbara toured the waterbirth suite, met with staff, and saw firsthand how the education and collaboration that began months earlier are continuing to shape maternity care.

And the nurses? They treated Barbara like royalty, welcoming her back with extraordinary warmth and appreciation.

This is why Barbara continues to travel, teach, consult, and advocate.

Over 109 days, she led 15 workshops and countless consultations, meetings, and parent classes across Russia, Spain, the UAE, Egypt, Israel, Turkey, Tanzania, and Nigeria.

But the real measure of that journey is what remains after she leaves: knowledgeable providers, stronger programs, greater confidence, and more opportunities for families to choose waterbirth.

For more than 40 years, Barbara’s mission has been not simply to teach waterbirth, but to give hospitals and birth professionals the evidence, protocols, and confidence to safely make it part of their own communities.

Barbara has always believed that access to high-quality birth education should not be determined by geography or resources. She continues to travel tirelessly, bringing evidence-based knowledge directly to communities where specialized waterbirth training may otherwise be difficult to access.

Joining Barbara was Agnes Ndunguru, affectionately known as Midwife Aggie, a Registered Midwife and founder and CEO of Audentia Birth Center in Dar es Salaam, Tanzania.

With more than a decade of experience, Aggie is a passionate advocate for the Midwifery Model of Care and for bringing Barbara’s teachings to more professionals and families throughout Africa.

This week, our director and founder Barbara Harper is in Tanzania, where the impact of more than four decades of teaching is beautifully coming full circle. Barbara, Midwife Aggie and Shereen Zarroug have created the Africa Birth Support Tour.

Barbara trained Midwife Aggie, who went on to establish Audentia Maternity Center in Dar es Salaam, Tanzania. Aggie created a space dedicated to respectful, family-centered care and the option of waterbirth.

Now, Barbara has returned to support Aggie, her team, and the families they serve.

Alongside them is Shereen, a birth coach and doula from Dubai whose help and assistance are invaluable during the tour. .

Together, they spent the day the first session of the tour for a Couple's Birth Class, were they guided more than 20 expectant couples through comfort measures, birth preparation, and the physiology of labor.

The workshop brought together families from Tanzania, the United Kingdom, Germany, South Africa, and beyond, reminding us that while cultures may differ, the experience of birth is universal.

👶💦**Did you know that the first randomized controlled trial of hospital waterbirth in the United States has been published?**💦👶

For decades, birth professionals have called for more high-quality research on hospital waterbirth.

That evidence continues to grow.

A newly published randomized controlled trial compared planned waterbirth with planned land birth among low-risk families in a U.S. hospital.

The results are very encouraging.

💧 Less Need for Pain Medication

Families planning a waterbirth experienced:

- 64% fewer IV narcotic pain medications

- 62% fewer epidurals

💧 Safe Outcomes

Researchers found no significant differences in overall maternal or neonatal complications between the waterbirth and land birth groups. Those planning a waterbirth also reported higher birth satisfaction and less stress during labor.

💧 Moving Waterbirth Forward

The authors conclude that these findings add to the growing body of evidence supporting waterbirth and provide support for hospitals to offer waterbirth as a pain relief option for carefully selected, low-risk families.

This landmark Committee Statement, "Partnering with Doulas in Clinical Settings," outlines how doulas, physicians, midwives, nurses, and hospitals can work together to provide more respectful, collaborative, and patient-centered care.

The guidance emphasizes that successful partnerships are built on mutual respect, communication, and a shared commitment to keeping birthing people at the center of every decision.

The Evidence Supports Doula Care

ACOG has long recognized that continuous labor support, including support from doulas, is associated with improved birth outcomes and should be considered an important part of maternity care.

As committee member Angela Daniel shared:

"This guidance demonstrates what is possible when we come together with mutual respect, a shared purpose, and a commitment to keeping birthing persons at the center of every decision."
