Anyone that is 16 & 17 will need a minor consent form to be filled out and notarized to be able to be tattooed. PIERCINGS:
Anyone under 18 will need a minor consent form filled out and notarized. For identification we accept State drivers license, passport, school ID, and birth certificates.
