This weekend, I was part of a group advancing our understanding of the airway. Under the leadership of , we are working together to understand the literature surrounding all aspects of breathing and functionally based surgical and nonsurgical therapies.

This ridge had... maybe 1mm thickness of tissue after extraction and bone grafting. Well, we are taught to make a 1mm dimple in soft tissue to create an "ovate" pontic. If I removed 1mm of tissue I would have been on BONE. No Bueno! So, to create a new "biologic" space for soft tissue, we remove 2mm of bone. We will have the lab "seal" the soft tissue at the margins with highly polished zirconia and leave a 2mm "gap" under the pontic so the body can heal with gingiva. The soft tissue will likely need at least 3 months to fully mature under the pontic. I will create more bleeding at the time of the bridge seat so the clot will fill the space under the pontic. In 3 months it will be a thing of beauty as the pontic will look like it's emerging from the tissue like a natural tooth, minimizing (if not eliminating) food impaction under the pontic while still allowing easy cleansing with floss.
