Managing a chronic condition like diabetes or heart failure means a lot of moving parts.

Appointments, medications, and specialists all add up.

A Chronic Condition Special Needs Plan (C-SNP) is a type of Medicare Advantage Plan built for that. Each plan focuses on a specific condition or group of related conditions.

Depending on the plan, a C-SNP may include:

- Doctors and specialists experienced with your condition
- A care coordinator to help manage your treatment
- Prescription drug coverage shaped around your needs
- A yearly limit on your out-of-pocket costs for covered Part A and Part B services

To qualify, you need Medicare Part A and Part B, you have to live in the plan's service area, and your condition must be confirmed by your medical records.

A C-SNP is one option among several, so it helps to compare before you decide.

Keep an eye on your mailbox this September! If you have a Medicare Advantage or Part D prescription drug plan, you should be receiving your Annual Notice of Change (ANOC) letter by September 30th. This letter explains what's changing with your plan starting January 1st. This includes things like your premium, deductible, drug coverage, which doctors or pharmacies are in network, and how benefit coverage is changing.

It's easy to set this letter aside, but taking a few minutes to read through it can save you from surprises come the new year. And if something in your ANOC catches your attention, remember that Medicare's Annual Enrollment Period runs from October 15th through December 7th, giving you time to compare plans and make a change if you need to.

Big news for Medicare Part D members

Starting July 1, 2026, a new pilot program called the Medicare GLP-1 Bridge may make certain weight loss medications more affordable for eligible beneficiaries.

Here are the highlights:
- Covered medications currently include Wegovy, Zepbound (KwikPen formulation), and Foundayo
- Eligible members would pay a flat $50 monthly copay, compared to typical retail prices that can exceed $1,000
- Eligibility is based on BMI plus related health conditions
- Prior authorization from a doctor is required
- The program runs through December 31, 2027

This is one of the most significant Medicare drug coverage changes in years. If you are already taking these medications, consult with your doctor to see if you may be eligible.
