Our staff provides community health, residential, & pregnancy/parenting services in Marion, Polk, & Yamhill counties in order to help support strong families & safe, healthy neighborhoods. Fostering Hope Initiative Community Health Workers (CHWs) provide system navigation to Citizen Waived Medical and non-Citizen Waived Medical members with the focus on engaging this population through Oregon Health Authority's auto-enrollment strategy in Marion and Polk counties. FHI's Community Health Workers:
· Provide one-to-one system navigation services in Marion and Polk County and assist individuals to enroll in health coverage.
· Employ strategies to overcome the barriers to utilizing Oregon Health Plan coverage and help navigate both Oregon Health Plan and other Coordinated Care Organization systems.
· Improve health literacy and navigation of health systems and health systems literacy.
· Collaborate with local community organizations and establish information sharing processes to facilitate outreach, enrollment, and system navigation.

At Fostering Hope Initiative, our Family Support & Connections program not only provides families with a reliable Family Advocate to help your family meet goals and find resources but also provides opportunities for community connection.

Be sure that if you are using a computer, you practice good safety tips and take breaks. Wear a properly fitted backpack that is not too heavy. And, incorporate moderate physical activity each day.

Changes are coming to OHP in 2027.
Be sure to have your mailing address up-to-date, check your mail frequently, and reply to any renewal letters. These changes will not impact all OHP members, but everyone should be on the lookout for important information that is mailed to them.
