FHI is a program of CCS that is comprised of CHWs/OHP Assisters. For more information contact: [email protected] or [email protected].

Our staff provides community health, residential, & pregnancy/parenting services in Marion, Polk, & Yamhill counties in order to help support strong families & safe, healthy neighborhoods. Fostering Hope Initiative Community Health Workers (CHWs) provide system navigation to Citizen Waived Medical and non-Citizen Waived Medical members with the focus on engaging this population through Oregon Heal

Our FHI buddy got to accompany our CHWs on some daily tasks yesterday!

A CHWs schedule is always changing and is often out in the community. Yesterday it included dropping off furniture to a family in our FHI truck!

FHI Community Health Worker, Orlando, wrapped up a parenting class series Make Parenting A Pleasure at Washington Elementary School in Woodburn. These parents were provided with the Early Learning Hub certification for their achievement in this class series.

Additional thanks to CHWs Leslie and April for supporting with several classes and facilitating alongside CHW Orlando. We have a great team at FHI and we are very thankful to these CHWs who are also parent educators in our community!

Congratulations to these parents and thank you for your participation and engagement!

We are very thankful for the families that have been attending our Family Connections groups in Marion, Polk, and Yamhill Counties over the past several years.

These groups are great opportunities to gather in community, meet other families and children, engage in a social-emotional activity as a family, and receive a free meal. If you have not attended one yet, we encourage you to try something new this summer!
