By the time primary MR looks bad on echo, the left ventricle has already remodeled in ways that surgical repair won't fully reverse.

In post-MI patients with MR, start your echo assessment with the wall.
Functional MR may be misread because the leaflets look structurally normal. The ventricle remodels and takes the leaflets with it, they get tethered and pulled into the chamber, and coaptation fails before they have a chance to close.
