Whatcoat United Methodist Dover will be offering vaccinations on Sunday, April 11, 2021, from 12-4 pm.
- You must be age 50 or older.
- Pre-registration is required.
- No walk-ins
- We will not know what vaccine will be allotted for the clinic until just before the event. If needed, we will schedule for the second vaccine if required.
- You must bring to your appointment the attached Vaccine Immunization Reporting Form. You will need to fill out the top box on the first page (i.e., name, address, etc.) and the question on the back of the form.
- Each person receiving the vaccine, and has medical insurance, will need to present their Medicare (red, white, and blue) card or prescription insurance card so your record can be updated. If you do not have health insurance you can still be vaccinated.
