APTA’s 2025 workforce survey puts numbers to something many of us have watched happen: 7.4% of PTs plan to leave or retire within two years, and another 16.2% plan to cut back. Most of the PTs I’ve watched leave still loved their patients. What wore them down was seeing more people every year and still falling behind on loans and rent.

Most PTs who write off virtual do it for telehealth’s reasons: the units, low reimbursement, and the sense that your time isn’t worth much on a screen.
None of that has anything to do with virtual PT.
If you’re judging one by the other’s rules, you’re ruling out something you haven’t actually tried.

Every PT with a following now started with zero followers, a bad camera, and no idea what they were doing. The only real difference between them and you is that they kept posting after the first ten people didn’t care.
You already did the hard part. Years of school, thousands of reps in the clinic. Showing up online is just letting people finally see it. So keep going!

This is telehealth: PT delivered virtually but still billed through insurance. That’s why it’s boxed into exact fifteen-minute units: insurance sets the unit, sets the rate, and reimburses it at a lower rate than the same time spent in person, because that’s how the system is built to control it.
Virtual PT is a different model. Your time isn’t tied to someone else’s units. In virtual cash-pay PT, you’re the one who decides what an hour of your work is worth.
So, what could that look like for you?
If you’re a virtual PT getting ready for a sales call, remember this: you already have the solution your client needs.
You’re not selling an evaluation or an hour of your time. You’re offering the value of your expertise and everything you already know how to do. When you truly own that, confidence comes naturally.
This is what we work on with virtual PTs: know your value, trust your expertise, and the rest will follow.
