Collaborative Bridges is proud to celebrate the launch of the PCC Community Wellness Center located inside the Collaborative Bridges Wellness Center at 4221 w. Lake Street on Chicago’s West Side.
This milestone reflects years of partnership to bring behavioral health, primary care, and future pharmacy services together in one location to expand access to care.
We appreciate the leaders who joined us, including Commissioner Tara Stamps, Alderman Emma Mitts, Alderman Jason Ervin, Nancy Cao of the Illinois Department of Healthcare and Family Services, Board Members Donald Dew and Kerri Brown.

🎤 "We meet our patients where they are, and we’re going to take them far."
That's from Beverly Echols, Ed.D., group director of educational services for both Hartgrove Behavioral Health System and Garfield Park Behavioral Hospital, and she and her team work very hard to help our patients keep up with their education while going through treatment.
Dr. Beverly (as the students call her 🙂) has more than 30 years of educational leadership experience across public, private and nonprofit schools, and she completely revamped our educational services department during her 18 months with us. Her team of licensed educators provide year-round, structured schoolwork time that's both intentional and personalized.
We partner very closely with schools across the region to make sure no kids fall behind. And if needed, we can help adults catch up, too! 🎓
