Sage Integrative Health is seeking a compassionate, organized, and grounded Patient Care Coordinator to join our team in Berkeley, CA.
This role is ideal for someone with experience in healthcare, wellness, or client-facing environments who feels called to integrative mental health, trauma-informed care, and psychedelic-assisted therapies.
As one of the first points of contact for individuals seeking care, the Patient Care Coordinator plays a vital role in helping patients feel welcomed, informed, supported, and connected to the services that best meet their needs. You’ll work closely with our clinicians, Clinic Director, and Operations team to support intake, onboarding, patient education, therapist-client matching, and continuity of care.

Please join us for an evening of connection, community, and celebration as we mark eight years of Sage Integrative Health.
In June of 2018, Sage opened its doors with a vision of creating a therapeutic community grounded in compassion, transformation, and equitable access to care. Eight years later, we are grateful to have served thousands of clients, trained practitioners from around the country, and built a vibrant community that continues to grow and evolve.
We would love to gather with you to celebrate this milestone, reconnect with familiar faces, meet new members of the community, and share our appreciation for everyone who has been part of Sage’s journey.
