🚀 The new CommUnityCare Health Centers Development, Operations, Population Health, and Eligibility Team is on the move—bringing healthcare access directly to our community!
At their first coordinated event at the Creedmoor Food Pantry, the team provided live screenings, eligibility assistance, appointment scheduling, service education, and MyChart support—ensuring families had the resources they needed to access care.
