Two prescriptions can contain the same active ingredient and still be very different medications to use.

Why?

Because a prescription includes more than the drug name.

The prescriber may need a different:
✔ Strength
✔ Dosage form
✔ Topical base
✔ Ingredient profile
✔ Method of delivery

That’s one of the reasons compounding exists.

FDA recognises that compounded medications can fill specific patient needs-for example, when a patient needs a formulation without a particular dye or needs a liquid because an appropriate approved liquid is unavailable.

But personalised does not mean automatically better. If an FDA-approved medication already meets the patient’s medical needs, that is generally the preferred option.

Same ingredient doesn’t always mean the same prescription.

Sometimes the answer may be no.

And that can be a sign of good pharmacy practice.

Compounded drugs are not FDA-approved, which means FDA does not review each compounded product for safety, effectiveness or quality before it is marketed. FDA therefore advises that compounded drugs should be used when an individual patient has a medical need that cannot appropriately be met by an available FDA-approved drug.

There are also legal and safety limits around which bulk drug substances can be used in compounding.

So before a prescription becomes a bottle, the questions matter:

✔ Is this appropriate for this patient?
✔ Is there evidence or a legitimate clinical rationale?
✔ Is the ingredient suitable for compounding?
✔ Could there be an interaction or safety issue?
✔ Is there already an appropriate approved option?

A personalised pharmacy shouldn’t say yes to everything.

Sometimes protecting the patient means knowing when to say no.

One of the most useful things to understand about eczema?

The treatment you need during a flare may not be the same treatment you need when your skin is calm.

During a flare:
The priority is controlling active inflammation and itch. Topical corticosteroids remain an important first-line option, alongside other prescription topical treatments depending on the patient and situation.

Between flares:
The focus shifts toward maintaining the skin barrier and preventing recurrence. Regular moisturisation remains fundamental, and some patients with recurrent eczema may be prescribed intermittent anti-inflammatory maintenance therapy.

And location matters too.

The skin on your face, hands, scalp and body may require different treatment approaches.

When your dermatologist needs a particular strength, dosage form or base that an available approved medication cannot provide, a compounded formulation may be considered.

Don’t wait for every flare to start from zero. Ask your dermatologist what your rescue plan AND your maintenance plan should look like.

A rash, stomach upset or other reaction after a medication does not automatically mean your child is allergic to the active drug.

Medicines can also contain inactive ingredients such as flavourings, colours, sweeteners and preservatives.

That doesn’t mean these ingredients are generally unsafe. But a specific patient may have a medically relevant allergy or intolerance to an ingredient in a particular product.

FDA guidance specifically gives the example of a patient with an allergy who needs a medication made without a particular dye contained in an FDA-approved drug.

The important part is:

Don’t guess which ingredient caused the reaction.

Talk to your pediatrician or pharmacist first. If the active medication is still appropriate but a specific inactive ingredient needs to be avoided, a patient-specific compounded formulation may sometimes be considered.

You finally find a prescription that should help your skin…

…but every time you use it, your face stings, feels greasy or becomes even more irritated.

The active medication may not always be the only thing worth discussing.

A topical medication also needs a vehicle or base-the cream, gel or ointment that carries the drug.

And that can affect:
✔ Texture
✔ Spreadability
✔ Drug release
✔ How the product feels on sensitive skin
✔ Whether you actually want to keep using it

So if a dermatologist wants to continue the same active medication but the available formulation isn’t suitable for a particular patient, compounding may allow a different prescribed base or dosage form to be prepared. FDA recognises compounding for individual patients when an available approved drug cannot meet the specific medical need.

Don’t automatically give up on the medication because you dislike the formulation. Talk to your dermatologist first.
