The egg you're hoping for in January is already developing right now.

An egg takes roughly 90 days to mature before it's released or retrieved. That means the way you're sleeping, eating, and handling stress this fall is part of the environment that egg is growing in.

Most women preparing for IVF, IUI, or trying on their own hear plenty about protocols and dates. Very few hear about the 90 days before.

That window is where we focus. At your fertility consultation we go through your cycle history, testing, timeline, sleep, and stress, then build a plan around your fertility team's calendar. Supportive care can't guarantee pregnancy. It can make sure you're not walking into your next cycle guessing.

90 days before IVF is the minimum. For some of you, it's not even close to enough.

An egg takes about three months to mature, so 90 days matters. But if you have PCOS, endometriosis surgeries, failed IVFs, poor sleep, low iron, or shorter cycles, you need more runway.

Clearing out what your body has too much of is fast. Building up what it's missing takes time. Prepare, don't react.

Eating less worked in our 20s and 30s. After 45, it stops working.

If you’re eating less and your belly keeps growing, you’re not doing it wrong. Your body changed the rules.

Here’s what I’m doing at 51:
Eating enough.

Start at least 90 days before your IVF cycle. I've been saying it for years, and I'm not changing my answer.

But for some of you, 90 days is your starting line, not your finish line. That's you if you have PCOS, a history of endometriosis surgeries, or more than one failed IVF. It's also you if you're not sleeping, running on high stress, low on iron, or your cycles have gotten shorter.

Here's why 90 days matters: an egg takes about three months to mature. That's the window where we support the egg you'll actually use.

And here's why some of you need more: clearing out what your body has too much of is faster than building up what it's missing. When you're depleted, rebuilding takes time. You can't rush it in the last few weeks before a transfer.

My approach is simple: prepare, don't react. I'd rather meet you months before your next transfer than the week before.

If your nose runs every October like clockwork, the next few weeks are your window to get ahead of it.

Fall allergy season in Wisconsin comes in waves. Ragweed hangs on until the first hard frost. Then the leaves come down, sit wet in the yard, and grow mold. Raking sends it straight into your face. Then the furnace kicks on and dries out every room in the house. By the end of October, plenty of people are waking up stuffed, rubbing itchy eyes, and going through a box of tissues a week.

Chinese Medicine has a useful way to look at this. Fall is Lung season, and in that system the Lungs handle more than breathing. They're in charge of your nose, your skin, and the barrier that decides what gets in and what your body overreacts to. When that barrier dries out and gets irritated, your body overreacts to things it used to handle fine.

In plain English: fall allergies are partly about what's in the air, and partly about how ready your defenses are when it shows up. That second part is something you can work on now.

Five things I tell patients every fall:

Moisten from the inside. Have warm drinks, soup, and cooked pears. Go easy on ice-cold drinks this season.
Rinse after you rake. Do a saline rinse made with distilled or boiled water, then shower after yard work so the leaf mold doesn't come to bed with you.

Humidify the bedroom. Aim for 30 to 50 percent humidity once the furnace is on. Going above 50 feeds mold, so don't overdo it.

Cover the back of your neck - I tell everyone this! Wear the scarf on windy days.

Protect your sleep. Your body does its repair work overnight, and a barrier that never gets a break stays reactive.

If fall knocks you flat every year, let's make this the year it doesn't.

The last appointment of my day never reschedules.

This is Roscoe. He's my shepherd mix rescue and he recently just turned eight.

He doesn't care how many patients I saw or how long the drive home was.

On my days off, he knows exactly where we're going as soon as I put my shoes on.

Pup cup time! Off to 7 Brew we go.

In the evening before dinner, it's the dog park.

He's not there to play with the other dogs. He's there to find every spot they p*ed on and p*e over it.

Every single one. Then we come home and he enjoys his Ollie like he just finished a hard day's work.

I used to think of this stuff as extra, something to fit in if I had time. Now I know it's part of taking care of myself. Watching him make his rounds at the park, or down a pup cup, brings me absolute joy.

I spend all day telling people the body heals best when the pressure comes off. Roscoe makes sure I take my own advice.

Waking up soaked at 2 AM is not random, and it is not just "getting older."

In your 40's, your internal thermostat gets touchy. The comfort zone it used to hold easily narrows to a sliver, so a shift that your body once ignored now reads as an emergency, and it slams the cool-down button. Heat surge. Sweat. Wide awake with your heart pounding.

Nighttime stacks the deck. If dinner was light on protein or the w**e was close to bedtime, blood sugar dips in the small hours. Your body releases stress hormones to correct it, and that surge is often the thing that flips the switch.

What helps steady the system: real protein at dinner, more space between w**e and bed, a genuinely cool room, and a nervous system that is not running on alert all day. A body that spends the day under pressure guards harder at night.
