Recurrent Pneumothorax – Overview and Thoracoscopic Management
Definition:
Recurrent pneumothorax refers to the reappearance of air in the pleural space after a previous episode has resolved, leading again to partial or complete lung collapse. It most commonly follows a primary spontaneous pneumothorax but can also occur in secondary cases (e.g., COPD, infections, or underlying lung disease).
Etiology & Risk Factors:
Rupture of subpleural blebs or bullae (most common)
Smoking
Tall, thin body habitus
Underlying lung diseases (COPD, cystic fibrosis, infections)
Inadequate pleural symphysis after first episode
Clinical Presentation:
Sudden onset pleuritic chest pain
Dyspnea (variable severity)
Decreased breath sounds on the affected side
Hyperresonance on percussion
In recurrent cases, symptoms are often similar but may present earlier due to patient awareness
Indications for Surgical Management:
Thoracoscopic intervention is recommended in:
Recurrent pneumothorax (same side)
First episode with persistent air leak (>5–7 days)
Bilateral pneumothorax
High-risk professions (pilots, divers)
Large bullae on imaging
Thoracoscopic Treatment (VATS – Video-Assisted Thoracoscopic Surgery):
Procedure Steps:
General anesthesia with single-lung ventilation
Three-port thoracoscopic approach
Identification of blebs/bullae (usually apical)
Bullectomy using endoscopic staplers
Pleurodesis to prevent recurrence, achieved by:
Mechanical pleural abrasion
Partial or complete pleurectomy
Chemical pleurodesis (e.g., talc) if needed
Advantages of Thoracoscopy:
Minimally invasive
Reduced postoperative pain
Shorter hospital stay
Faster recovery
Low recurrence rate (≈1–5%)
Postoperative Care:
Chest tube drainage with suction initially
Monitoring for air leak
Pain control
Early mobilization
Complications:
Persistent air leak
Bleeding
Infection
Recurrence (rare after proper pleurodesis)
Conclusion:
Thoracoscopic management (VATS) is the gold standard for recurrent pneumothorax. It effectively treats the underlying cause (blebs/bullae) and significantly reduces recurrence through pleurodesis, with excellent patient outcomes and minimal morbidity.

اخواني اعطوني من وقتكم بضع دقايق واقرؤوا للأخر ، لتصل الفكرة

قصة الخروف الذي صار كلباً ( قصة من وحي الواقع )
كان لأحد الأشخاص
خروفاً أراد بيعه فأخذه إلى السوق فرءآه أربعة لصوص
إتفقوا أن يسرقوا منه الخروف بأسلوب ذكي ...
فتقاسموا الجلوس على جانب الطريق المؤدية للسوق التي سيمر منها صاحب الخروف
فجلس الأول في بداية الطريق ..

المؤدي للسوق وجلس الثاني في ربع الطريق وجلس الثالث بعد منتصف الطريق وجلس الرابع قبل نهاية الطريق بقليل .
فمر صاحب الخروف من جانب اللص الأول وألقى عليه السلام فرد اللص السلام وبادره بالسؤال :
لماذا تربط هذا الكلب وتقوده خلفك ..
فإلتفت صاحب الخروف إلى اللص وقال له :

ا ليس كلباً !!إنه خروف سأذهب لبيعه في السوق
ثم تركه وإنصرف وبعد مسافة إلتقى باللص الثاني وإذا به يسأله :لماذا تربط هذا الكلب وتقوده خلفك! فنظر إلى اللص وقال له : هذا خروف وأنا ذاهب لأبيعه في السوق وتركه وإنصرف لكن الشك بدأ يتسرب إلى قلبه فأخذيتحسس الخروف ليتأكد هل هو كلب فعلاً

كما سمع من ذلك الرجلان
أم أنه خروف كما يعتقد هو ؟!!

وبعدمسافة إلتقى باللص الثالث وإذا باللص يسأله نفس الأسئلة السابقه :
لماذا تربط الكلب خلفك !
فأندهش صاحب الخروف وزادت حيرته ... ونظر إلى اللص ثم انصرف ولم يجبه على سؤاله لأنه بدأ يتأكد أنه يقود كلباً وليس خروفاً

وبعد مسافة إلتقى باللص الرابع فسلم عليه وباشره اللص قائلا : مابك يارجل تربط الكلب وتقوده خلفك !! هنا تأكد صاحب الخروف أنه يقود كلباً وليس خروفاً فليس من المعقول أن يكون الأربعة الأشخاص كاذبون ؟!
ثم إلتفت إلى اللص وقال له :

لقد كنت في عجلة من أمري فأعتقدت أن هذا الكلب خروفاً

فربطته لأذهب به إلى السوق لأبيعه
ولم يتبين لي أنه كلباً
إلا الآن
ثم فك وثاق الخروف
وأطلق سراحه وعاد مستعجلاً إلى بيته يبحث عن خروفه ...

فأخذ اللصوص
الخروف وأنصرفوا وهم يتهامسون بسرور وغبطة .

الدرس والعبرة من هذه القصة :

١- هكذا تتم صناعة الرأي العام والتضليل من قبل المحترفين عبر وسائل الأعلام والتواصل الإجتماعي والوسائط على جميع المستويات الشخصية والعربية والعالميه ويفعلون لأجل ذلك كل ما يمكن من تزييف وقلب للحقائق وصناعة المشهد التمثيلي للخداع واللعبة بالعقول..
وهذا ما يحصل معنا الأن ولا تنخدعوا حتى بمن كانوا للأمس القريب اصدقائنا وقد انخرطوا اليوم بلعبة التضليل والتأثير على الرأي العام لنصل نحن الى القول هل يمكن لكل هؤلاء ان يكونوا يكذبون لنبدء بالتشكيك بكل ما نعتقده ونؤمن به ،
وانتم اهل الوعي والبصيرة والثبات
