We’re Hiring: Care Coordinator

We are looking for a dedicated Care Coordinator with a background in nursing, aged care, or healthcare administration.

The role involves coordinating supports, communicating with participants and support coordinators, and assisting with documentation such as risk assessments, support plans, and incident reports.

Work Schedule: 6:00 AM – 3:00 PM (PH Time) | Onsite
Note: The Care Coordinator may occasionally need to respond to emergency calls outside working hours, strictly for urgent matters.
