Three things often get tangled together in perimenopause, and they're worth pulling apart.
One: shifting s*x hormones. Estrogen, progesterone, and testosterone all start to decline.
Two: thyroid function. The thyroid doesn't operate in a vacuum. As s*x hormones drop, thyroid signaling and conversion can shift along with them.
Three: inflammation and autoimmune activity. The hormone shifts of perimenopause can ramp up inflammation, which is part of why this is a common window for autoimmune conditions like Hashimoto's (where the immune system attacks the thyroid) to show up or worsen.
The result: a lot of women in their 40s end up with the same symptom cluster — fatigue, weight gain, sleep problems, mood changes — and a thyroid panel that comes back "normal." The thyroid alone isn't the whole picture. The s*x hormones aren't either. Both need to be looked at together.

Most women are taught that menopause shows up around 50 and that's when "hormone stuff" starts. The reality is more interesting — and more useful to know early.
Perimenopause is the years of hormone transition leading up to menopause (when periods stop for 12 consecutive months). On average, it begins around age 43, about 7 years before menopause itself. But it can start as early as 35.
During those years, estrogen, progesterone, and testosterone all begin to decline. That decline is gradual but real, and it's often what's behind the fatigue, sleep disruption, brain fog, mood shifts, and libido changes that women in their late 30s and early 40s start noticing.
