Provider Reminder: Metabolic Monitoring Matters
Patients with severe mental illness (SMI) and those prescribed antipsychotic medications may face increased risks for metabolic syndrome, diabetes, and cardiovascular disease.
Routine metabolic monitoring is a simple but important step that supports early identification, timely intervention, and better long-term patient outcomes.
For providers and billing teams, this is also a reminder to make sure these services are:
✅ Ordered when clinically appropriate✅ Completed according to monitoring guidelines✅ Documented clearly in the medical record✅ Billed correctly based on payer policy and covered services✅ Coordinated between behavioral health, primary care, and lab services when needed
When care is properly monitored, documented, and billed, practices can support quality patient care while also protecting appropriate reimbursement.
BAC-Office encourages providers to review their current workflows for patients with SMI and/or antipsychotic prescriptions to ensure metabolic testing is not being missed.

Hawai‘i CCS Contract Update
The Community Care Services (CCS) transition timeline has been updated.
AlohaCare was awarded the statewide CCS contract, but the transition start date is now expected to begin October 1, 2026 instead of the previously expected July 1, 2026 date.
Providers should continue monitoring official Med-QUEST, AlohaCare, and current CCS plan communications for transition guidance, authorization updates, billing instructions, and network notices.
BAC-Office will continue watching for updates that may impact billing, credentialing, and provider workflow preparation.

We want to make you aware of a recent Medicare update that may impact patient eligibility records and claim processing. On March 11, 2026, CMS notified Medicare plans that approximately 1.3 million Medicare Beneficiary Identifier (MBI) reassignments were processed with an effective date of April 14, 2026. These newly reassigned MBIs are believed to be connected to a recent data security breach.
As a precautionary measure, we strongly recommend that all offices verify Medicare eligibility and confirm each patient’s current Medicare Beneficiary Identifier (MBI) before rendering services and again before claim submission. Using an outdated or incorrect MBI may result in eligibility rejections, claim denials, and reimbursement delays.
Using an outdated or incorrect Medicare ID can result in:
Claim denials
Eligibility rejections
Delays in reimbursement
Coordination of benefits issues
Patient billing complications
Recommended Best Practices
To help minimize denials and payment delays, we recommend the following:
Verify Medicare eligibility for all Medicare patients prior to appointments and again before claim submission when possible.
Confirm the patient’s current Medicare Beneficiary Identifier (MBI) directly through your eligibility verification system or payer portal.
Ask patients to provide their most current Medicare card at check-in.
Update patient demographics and insurance information immediately if changes are identified.
Re-run eligibility checks for patients who have not been seen recently or who have experienced recent insurance changes.
Why This Matters
Medicare may update or replace beneficiary identifiers, and in some situations patients may not realize their information has changed. Submitting claims with inactive or incorrect Medicare IDs can increase rejection rates and slow reimbursement timelines.
Taking a few extra moments to verify eligibility can significantly reduce preventable denials and administrative rework.
