📄💻📧We’ve recently had an increase in patient forms being dropped off and wanted to share a quick reminder with our Northpointe families.

All forms are carefully completed and signed by your child’s provider (often during or alongside well visits), so we kindly ask that you allow 3 business days for completion.

We absolutely understand that many forms are time-sensitive, and we always work to get them done as quickly as possible! To make things easier, we offer:
✔️ Email delivery
✔️ Direct faxing to schools/sports programs
✔️ In-office pickup

MDHHS recommends early measles vaccine doses in areas of the state with possible community transmission Michigan's Women, Infants & Children program, providing supplemental nutrition, breastfeeding information, and other resources for healthy mothers & babies.

We will be open until noon today to give our staff the opportunity to observe Good Friday.
Saturday will be our normal hours of 8am-noon.
