SWACH
SWACH partners to improve health in Washingtonโs Clark, Klickitat and Skamania Counties and the tribal communities of the region.
SWACH is headquarters for improving community resiliency and health in Southwest Washington. Leaders in innovative community engagement, we advance health as a consideration for all policy decisions, and convene a wide range of voices to discuss difficult issues. We are the Accountable Community of Health (ACH) for Southwest Washington, and implement the Medicaid Transformation projects in our region.

Belonging supports health.
When people feel included, valued, and connected, communities are stronger and healthier. Social belonging can support mental well-being, reduce stress, and help people feel more connected to the resources and relationships around them.
Equity helps make belonging possible. When communities remove barriers and ensure people have fair access to resources, more people have what they need to thrive.
Together, we can build healthier, more equitable communities where everyone belongs.

Community wellness is about more than doctor visits.
It also includes the everyday supports that help people live healthier, more stable lives, like housing, food, transportation, mental health support, social connection, preventive care, culturally responsive services, and local resources.
When communities have what they need, people have more opportunities to be healthy.
Together, we build healthier communities.
