Before Johnny and Roy there was:

In the late 1960s, if you were Black in Pittsburgh’s Hill District and suffered a heart attack or a gunshot wound, the city’s “emergency response” was a police paddy wagon. No oxygen. No medical training. No life-saving intervention.

The city didn’t view Black lives as worthy of a medical infrastructure, so Freedom House Enterprises, a local organization born from the community’s own survival instincts decided they were done waiting.

They just built a better system.

They partnered with Dr. Peter Safar and Dr. Nancy Caroline to recruit residents from the Hill District men and women whom the establishment had dismissed as "unemployable" and transformed them into the nation’s first true paramedics.

These men didn't just drive ambulances; they brought the hospital to the streets. They performed the first tracheal intubations in the field, utilized the first mobile EKG machines to relay data to hospitals, and pioneered field-based drug interventions. They turned the ambulance from a transport vehicle into a mobile emergency room.

Their success was undeniable, and that was the problem.

The medical establishment didn't applaud this breakthrough. They saw a Black-led, community-owned operation that was saving more lives and outperforming the city’s own services. They felt threatened by this shift in power. By 1975, the city dismantled Freedom House, seized the revolutionary protocols and training standards they had forged, rebranded them as "city services," and systematically pushed the people who built them out of the industry.

We are often told that the modern EMS system was a gift from a benevolent government. That is a lie. It was a hijacked innovation. Every life saved by an ambulance in America today is being saved by protocols and systems that were forged in the Hill District.

Johnny Gage Answered His Last Tones Today.

I grew Up near Randy, when he lost his home in one of the Malibu fires years ago, we were able to help him out with some farm animal donations

Today, the EMS community lost more than an actor.

We lost a man whose work inspired generations of firefighters, EMTs, and paramedics.

Rest in peace, Randolph Mantooth—forever known to millions as Firefighter-Paramedic John Gage from Emergency!.

For those outside our profession, Emergency! 🚨 may have been just another television show.

But for those of us who answer 911 calls, it was something much bigger.

When Emergency! premiered in 1972, organized paramedic systems were still in their infancy.
Across much of America, advanced prehospital care simply didn’t exist. Some would even say you had a better chance of survival in a war zone than in the streets of America 🇺🇸

Ambulances were often little more than transportation, and lifesaving interventions were reserved for the hospital.

Emergency! changed that.

Working closely with medical experts, including Dr. James O. Page and Dr. Walter Graf, the show’s producers made a deliberate effort to portray paramedicine accurately. Week after week, viewers watched Johnny Gage and Roy DeSoto bring advanced medical care directly to their patients.

Millions of Americans saw CPR, cardiac monitoring, defibrillation, airway management, trauma care, and the concept of specially trained paramedics for the very first time.

The show’s popularity did more than entertain—it educated. It sparked conversations in city halls, fire departments, and state legislatures.

Communities began asking, “Why don’t we have paramedics like Johnny and Roy?”

Many leaders have credited Emergency! with helping build public support for creating and expanding EMS systems throughout the United States.

Its influence reached beyond the public.

Countless EMTs, paramedics, firefighters, nurses, and physicians can trace their career choice back to sitting in front of a television, watching Station 51 answer another call.

I am one of those people.

Every time we climb into an ambulance, start an IV, interpret an ECG, calm a frightened patient, or work alongside our brothers and sisters in the fire service, we are part of a profession that Emergency! helped bring into America’s living rooms.

Randolph Mantooth didn’t just play a paramedic. He became one of EMS’s greatest ambassadors, spending decades supporting first responders, attending EMS conferences, advocating for firefighter health and safety, and never forgetting the community that embraced him.

His legacy isn’t measured in television ratings.

It’s measured in the lives saved by the men and women he inspired to dedicate themselves to this profession.

Thank you, Randolph, for giving us Johnny Gage.

Thank you for helping America believe in paramedics.

And thank you for inspiring generations of us to answer the call.
