Sacramento County - Government ’s 2026-27 budget fully funds Community HealthWorks, preserving a proven model that connects people to housing, health care, food, employment support, and other stabilizing services.

“In the face of many difficult budget decisions, the Sacramento County Board of Supervisors recognizes that Community HealthWorks operates an effective model that continues to produce strong results,” said Kelly Bennett, Founder and CEO of Community HealthWorks.

Over the past three years, CoHeWo has helped 1,362 people move into permanent housing, 190 into transitional housing, and 1,261 into shelter.
These investments lead to trusted, community-based support.

CoHeWo is here to help our community maintain access to nutritious food across Sacramento County.

A new round of reductions to CalFresh eligibility took effect last week. New work requirements, reduced exemptions for some of California's most vulnerable residents, and stricter rules for some individuals mean that many people who relied on food assistance may now face new barriers.

Our community health workers have helped hundreds of Sacramento residents navigate CalFresh enrollment. As these changes take effect, our team is ready to help people understand what’s changed, see what programs they may be eligible for, and get support applying for the food assistance and resources they need.

This Asian American and Pacific Islander Heritage Month, we're celebrating the members of our team who bring lived experience and cultural understanding to the communities we serve every day.

Research shows that health systems work better when the people navigating them reflect the communities they serve. Our staff of Community Health Workers offers services in 12 languages and meets people where they are, making care more accessible, culturally responsive, and effective.

For many of the people we serve, mental health might be connected to housing instability, other health issues, gaps in healthcare coverage, or difficulty navigating complexities in the system.

That's why our Community Health Workers show up every day, meeting people where they are and supporting them on a path that suits their unique needs.

During Mental Health Awareness Month, we celebrate our CHWs for their essential role in connecting people to care, housing, and services that support long-term stability and well-being.
