Here's a picture of a commercial egress stair that we recently designed and engineered for a client in White Oak. We also acted as the owner's rep on this project assisting with contractor evaluation and project coordination to minimize disruption to the existing business operations.

These bolts on the emergency egress stair are installed cockeyed and not to manufacturer specifications. Having qualified post construction inspections is critical to make sure the work is installed correctly. These are the types of oversights that can lead to disaster… Inspected and rejected!

Successful projects are more than just a good design and the right engineering. Quality construction management and inspection are also very important, even the best designed systems can fail if they are improperly installed. If you zoom in on the large vertical posts in the photo you can see they wooden post was notched for the metal knife plate to go into the column and be bolted. Several of the posts were not properly installed, hopefully the construction inspector for this project has this corrected or verified with the structural engineer that the alternate attachment is ok.
