Meet Ivanna Fuentes, a Licensed Clinical Social Worker dedicated to helping patients feel supported and understood in their mental health journey. Fluent in Spanish and originally from Santo Domingo, Dominican Republic, she is passionate about providing accessible, culturally responsive care to adolescents and adults from diverse backgrounds.

Ivanna received a Masters degree in Social Work from Utah State University. During her practice, Ivanna worked with middle school children, adolescents and adults.

Meet Hailey Spillane, a Psychiatric Mental Health PA dedicated to helping patients feel supported, informed, and empowered in their care. Fluent in both English and Spanish, Hailey is committed to making quality care accessible to a diverse community.

Hailey brings experience in family practice and urgent care, with special interests in mental health and caring for patients and families within the LGBTQ community. She is passionate about preventative care and helping patients manage chronic conditions, while promoting wellness for individuals and families of all ages.

Originally from Michigan, Hailey moved out West to be closer to the mountains and eventually settled in Ogden, Utah. Outside of work, Hailey enjoys rock climbing, kayaking, gardening, skiing, and hiking with her two dogs.

Meet Tyler Farnsworth, a Psychiatric Mental Health Nurse Practitioner providing compassionate, evidence-based care for patients across the lifespan.

Tyler completed his Doctor of Nursing Practice at The Ohio State University and an advanced fellowship in Child & Adolescent Psychiatry at Primary Children’s Hospital, where he gained specialized experience working with children, adolescents, and families. He has a strong interest in supporting individuals with substance use disorders and other complex mental health needs.

He is committed to creating a collaborative, respectful space where patients feel heard and supported in their care.

Outside of work, Tyler enjoys skiing, hiking, paddle boarding, and camping with his two Australian Shepherds. Alaska remains his favorite place in the world, and he returns there each year.
