Western Illinois Area Agency on Aging is hosting it's inaugural Trivia Night Fundraising "Aging is Anything But Trivial" on Friday May 15th at Bally's Casino.

For our friends in the Galesburg area. This is a chance to support caregivers and those living dementia.

We are seeking a mission minded individual to join our team to help older adults transition from hospital to home! You’ll:

-Complete 4-5 Enhanced Choices for Care screens weekly

-Make post-hospital follow-up calls and home visits

-Coordinate care with medical providers, family, and community resources

-Support patients for 4–6 weeks to ensure a smooth recovery

Requirements:

-Bachelor's in social work, nursing, or related field (or RN with experience)

-Strong communication and time management skills

-Driver’s license & reliable vehicle
