Not all Myers Cocktail IV infusions are created equal.

Many IV drip businesses offer a standard Myers infusion containing:
• Magnesium – 96.5 mg
• Vitamin C – 644 mg
• Vitamin B12 – 1,000 mcg
• B Complex Vitamins

At Direct Functional Medicine, we've designed our Myers infusion to provide significantly more nutritional support:

✓ Magnesium – 1,000 mg
✓ Vitamin C – 2,000 mg
✓ Methylated Vitamin B12 – 5,000 mcg
✓ Our specialized B Complex blend

Why does this matter? Higher therapeutic doses and methylated nutrients may provide better support for energy, recovery, immune function, hydration, and overall wellness—especially for those who struggle with nutrient deficiencies or absorption issues.

Our patients consistently tell us they can feel the difference.

If you're looking for more than just a basic IV drip, come experience the Direct Functional Medicine Myers Cocktail and see what a difference the right formulation can make.

Could FSH Be the Missing Hormone in Alzheimer's Prevention for Women?

For years, the conversation around menopause and brain health focused almost entirely on declining estrogen. But emerging research is showing that another hormone may play a critical role: Follicle Stimulating Hormone (FSH).

A recent study published in Frontiers in Aging Neuroscience found that higher FSH levels were associated with greater amyloid deposition in the brain and worse cognitive performance in postmenopausal women. Researchers found that women with Alzheimer's disease had the highest FSH levels, while cognitively healthy women had the lowest. This suggests that rising FSH may not simply be a marker of menopause—it may be actively involved in the disease process.

At the same time, newer research is challenging decades of "normal" estradiol reference ranges used for menopausal women. Emerging data suggests that maintaining estradiol levels closer to physiologic ranges—often around 100-150 pg/mL for many women—may provide greater support for brain metabolism, neuroplasticity, mitochondrial function, and protection against neurodegeneration than the traditionally accepted low postmenopausal levels.

The takeaway?

✅ Alzheimer's prevention may be about more than simply replacing estrogen.

✅ Monitoring and controlling the rise in FSH may become an important target for preserving cognitive health.

✅ Optimizing estradiol levels may help support the brain during the vulnerable perimenopause transition.

This is why hormone therapy should never be a "one-size-fits-all" approach.

A comprehensive evaluation should consider:

• Estradiol levels
• FSH levels
• Progesterone balance
• Thyroid function
• Inflammation markers
• Insulin resistance and metabolic health
• Genetics and methylation pathways
• Nutrient status and detoxification capacity

Perimenopause is not just about hot flashes and irregular periods. It is a critical neurological transition that may influence brain health for decades to come.

If you're experiencing brain fog, memory changes, fatigue, anxiety, sleep issues, or other symptoms of perimenopause, don't settle for being told your labs are "normal."

The goal isn't simply surviving menopause.

The goal is protecting your brain, your metabolism, and your future health.

🔥 Benefits of Infrared Sauna Therapy 🔥

Infrared sauna is more than just relaxation—it’s a powerful wellness tool that supports the body on multiple levels.

Unlike traditional saunas that heat the air around you, infrared heat penetrates deeper into the body, helping you sweat at a cellular level.

Some potential benefits include:

🌿 Supports natural detoxification
💪 May help reduce muscle and joint pain
💓 Supports healthy circulation and cardiovascular function
😌 Promotes relaxation and stress reduction
✨ May improve skin clarity and overall glow
😴 Can support better sleep and recovery

I'm on Testosterone, So Why Do I Still Feel Terrible?"

Many men assume that if their testosterone level is "normal" or they're taking testosterone replacement therapy (TRT), all of their symptoms should disappear.

But what happens when you're still struggling with:

❌ Irritability and mood swings
❌ Loss of morning erections
❌ Erectile dysfunction
❌ Increased abdominal fat
❌ Low motivation and drive
❌ Brain fog
❌ Fatigue
❌ Difficulty building muscle
❌ Decreased libido

The answer may not be a lack of testosterone—it may be how your body is processing and utilizing hormones.

Testosterone doesn't work in isolation.

A portion of testosterone naturally converts into estrogen through a process called aromatization. While estrogen is essential for men, excessive estrogen levels or poor estrogen metabolism can contribute to symptoms that many men mistakenly attribute to low testosterone.

A comprehensive hormone evaluation should look beyond total testosterone and assess:

✔️ Estradiol levels
✔️ Estrogen metabolism and detoxification pathways
✔️ Aromatase activity
✔️ Liver function
✔️ Methylation pathways
✔️ Thyroid function
✔️ Insulin resistance and metabolic health
✔️ Inflammation
✔️ Gut health
✔️ Nutrient deficiencies
✔️ Environmental toxin exposure

Many men with abdominal weight gain have increased aromatase activity because fat tissue converts testosterone into estrogen. The result can be a vicious cycle of lower free testosterone, higher estrogen, worsening insulin resistance, and continued symptoms.

In addition, poor methylation, sluggish liver detoxification, gut dysfunction, and chronic inflammation can impair the body's ability to properly metabolize and eliminate estrogen metabolites.

The goal isn't simply achieving a testosterone number on a lab report.

The goal is optimizing the entire hormonal environment so testosterone can work effectively while maintaining healthy estrogen balance.

If you're on testosterone and still don't feel like yourself, it may be time to stop focusing on testosterone alone and start looking at the bigger picture.

Hormone optimization is about more than replacing hormones—it's about understanding how the entire body works together. 💙

Why COMT Matters in Hormone Replacement Therapy (HRT)

If you've ever wondered why one woman feels amazing on hormone therapy while another develops headaches, anxiety, breast tenderness, irritability, or worsening symptoms, the answer may lie in a gene called COMT.

COMT (Catechol-O-Methyltransferase) is an enzyme responsible for helping your body break down and clear estrogen metabolites, as well as neurotransmitters like dopamine, epinephrine, and norepinephrine.

When COMT is functioning more slowly, estrogen metabolites can accumulate, leading to:

⚠️ Increased estrogen sensitivity
⚠️ Anxiety or mood changes
⚠️ Breast tenderness
⚠️ Headaches or migraines
⚠️ Sleep disturbances
⚠️ Difficulty tolerating hormone therapy

But COMT is only one piece of the puzzle.

Successful hormone replacement therapy requires understanding:

✅ Estrogen detoxification pathways (2-OH, 4-OH, and 16-OH metabolism)
✅ Methylation function (MTHFR, MTHFD1, COMT and other genes)
✅ Liver health and toxin burden
✅ Gut health and the estrobolome
✅ Thyroid function
✅ Nutrient status (B vitamins, magnesium, zinc, SAMe, choline)
✅ Inflammation and oxidative stress

Many women with fibroids, endometriosis, heavy periods, estrogen dominance symptoms, or difficulty tolerating hormones may benefit from a deeper evaluation of these pathways rather than simply increasing or decreasing hormone doses.

Hormone therapy isn't just about replacing hormones—it's about helping your body use, metabolize, and eliminate them properly.

A personalized approach can often mean the difference between feeling worse and finally feeling like yourself again.

Perimenopause: Birth Control vs. Bioidentical Hormone Therapy — What's the Difference?

Many women are offered birth control pills when they begin experiencing symptoms of perimenopause such as irregular periods, mood swings, hot flashes, brain fog, and sleep disturbances. While birth control can sometimes mask symptoms, it's important to understand that it works very differently than bioidentical hormone therapy.

💊 Birth Control Pills
Birth control works by shutting down communication between the brain and ovaries. The synthetic hormones suppress ovulation and significantly reduce your body's natural production of estrogen, progesterone, and testosterone. While this may regulate cycles or reduce symptoms temporarily, it does not support your body's natural hormone production.

Birth control can also:
• Increase thyroid-binding proteins, which may lower available free thyroid hormone and contribute to symptoms such as fatigue, hair loss, weight gain, and brain fog.
• Increase the body's demand for nutrients involved in methylation, including folate, vitamin B6, vitamin B12, magnesium, and zinc.
• Potentially affect detoxification pathways and neurotransmitter production in susceptible individuals.

🌱 Bioidentical Hormone Therapy
Bioidentical hormones are designed to match the hormones your body naturally produces. Rather than suppressing ovarian function, they can help support hormone levels as natural production begins to fluctuate and decline during perimenopause.

Benefits may include:
• Improved sleep
• Better mood stability
• Reduced hot flashes and night sweats
• Enhanced cognitive function
• Support for bone, heart, and metabolic health
• More physiologic hormone replacement tailored to your individual needs

The goal isn't simply to "cover up" symptoms—it's to understand what's changing in your body and provide support that aligns with your biology.

Every woman's situation is unique, and the best approach depends on her symptoms, age, health history, and goals.

If you're struggling with perimenopause symptoms and wondering about your options, let's discuss a personalized approach to help you feel like yourself again.

🔥 Could Your "Allergies" Actually Be MCAS? 🔥

Do you experience symptoms that seem unrelated but keep showing up over and over?

✔️ Flushing or feeling hot
✔️ Hives, itching, or rashes
✔️ Sinus congestion
✔️ Brain fog
✔️ Anxiety or panic-like symptoms
✔️ Dizziness or POTS
✔️ Digestive issues (bloating, diarrhea, nausea)
✔️ Headaches or migraines
✔️ Sensitivity to foods, medications, fragrances, or chemicals

These symptoms may be related to Mast Cell Activation Syndrome (MCAS), a condition where mast cells release excessive amounts of histamine and other inflammatory chemicals.

Mast cells are part of your immune system and are found throughout the body. When they become overactive, they can affect multiple systems at once—including the skin, gut, brain, lungs, cardiovascular system, and nervous system.

Common triggers may include:
🔹 Stress
🔹 Hormonal changes
🔹 Mold exposure
🔹 Infections
🔹 Environmental toxins
🔹 Certain foods
🔹 Temperature changes

MCAS is often overlooked because symptoms can mimic allergies, IBS, anxiety, chronic fatigue, autoimmune conditions, or even hormone imbalances.

A functional medicine approach focuses on identifying WHY mast cells are overreacting rather than simply masking symptoms. Addressing underlying triggers such as gut dysfunction, chronic infections, mold exposure, inflammation, nutrient deficiencies, and hormone imbalances can often make a significant difference.

If you've been told "everything looks normal" but you still don't feel well, it may be worth exploring whether mast cell activation is playing a role.

Massage Therapy & Ehlers-Danlos Syndrome (EDS): More Than Just Relaxation

Living with Ehlers-Danlos Syndrome can mean dealing with chronic muscle tension, joint instability, pain, fatigue, headaches, and frequent muscle spasms. While massage therapy won't change the underlying connective tissue disorder, it can be a valuable tool for symptom management when performed by a therapist knowledgeable about EDS.

Benefits of Medical Massage for EDS:
✅ Reduces muscle tension caused by joints working overtime to stabilize the body

✅ Helps decrease pain from muscle guarding and trigger points

✅ Improves circulation and lymphatic flow

✅ May reduce headaches and neck tension

✅ Promotes relaxation of the nervous system, helping decrease stress and improve sleep

✅ Can improve body awareness (proprioception), which is often impaired in EDS

✅ Supports recovery from daily activities and exercise

Research suggests that individuals with hypermobility disorders often develop significant muscle tension as the body attempts to compensate for unstable joints. Addressing these compensatory patterns can help reduce pain and improve function.

At Direct Primary Care Mebane, our medical massage approach focuses on supporting the body's biomechanics, reducing muscle compensation patterns, and helping patients move more comfortably while respecting the unique needs of connective tissue disorders like EDS.

💙 If you have EDS and struggle with chronic pain, muscle tightness, headaches, or fatigue, medical massage may be an important part of your comprehensive care plan.

I saw chiropractors for 16 years for lower back pain. I saw a spine specialist at Duke 14 years ago. I was told I had a torn disc in my lumbar spine and could not exercise anymore. I never stopped exercising. The spine doctor gave me bad advice. I had been to massage therapists but no one ever thought to massage my psoas muscle. Once I started getting psoas massages I stopped needing the chiropractor. I am lifting the same amount of weight I did in my 20's prior to my back issues. If you are suffering from low back pain please call our office at 919-568-0004 to schedule a medical massage.
