What Are Surveyors Looking For in Activity Documentation?

Activity documentation is not just about proving that a calendar exists.

Surveyors are often looking for evidence that residents are receiving meaningful, individualized opportunities for engagement based on their interests, preferences, abilities, culture, lifestyle, and needs.

That means strong activity documentation should help show:

✔ The resident was assessed for interests and preferences
✔ Activities were offered based on the resident’s abilities and choices
✔ Adaptations were made when needed
✔ The resident’s response was observed and documented
✔ Care plans reflected meaningful engagement goals
✔ Staff followed through with planned interventions
✔ The activity program supported quality of life, not just attendance

A sign-in sheet may show who was present.
