Medicare Part A and B are a great start. But they were never designed to cover everything, and most people do not find that out until they are staring at a bill they did not expect.
Here is what original Medicare does not tell you upfront.

Did you know your Medicare Part D drug costs can change three different times throughout the year? Most people only find out when they notice their copays suddenly look different at the pharmacy counter.
Here is a simple breakdown of the three stages of Medicare Part D coverage for 2026.
𝗦𝘁𝗮𝗴𝗲 𝟭 is the deductible stage. You pay 100% of the cost of your covered prescriptions until you meet your plan's deductible. In 2026, the maximum deductible is $615, though many plans offer a lower deductible or even $0 to start.
𝗦𝘁𝗮𝗴𝗲 𝟮 is the initial coverage stage. After your deductible is met, you pay 25% of your drug costs. Your Part D plan picks up 65%, and the drug manufacturer covers the remaining 10%. This continues until your total out-of-pocket spending on covered drugs reaches $2,100 for the year.
𝗦𝘁𝗮𝗴𝗲 𝟯 is catastrophic coverage. Once your spending hits that $2,100 cap, your cost drops to $0 for the rest of the calendar year. Medicare, your plan, and the drug manufacturer split everything from that point forward.
One more thing worth knowing. The old coverage gap, once called the donut hole, is officially gone. Starting in 2025, the Inflation Reduction Act eliminated it, making the Part D benefit much simpler and more predictable than before.
