This case was previously initiated by a general dentist. The general dentist found the or***ce to the MF2 but couldn’t navigate it. The dentist also had problems getting a proper working length on the palatal, likely due to the open apex.

I also had difficulty with the open apex… I got a good length with the LSX, but the Simplifil wouldn’t fit because of the irregular taper in the coronal half of the canal… because I didn’t want to remove excessive dentin, I decided to place an apical plug. As you can see I tried to compensate for a minor void but ended up with a separate minor void in the final obturation. No worries though, as the apical obturation is good! Look at the lateral canal on the MF2 canal!

Finished this case today. Six canals! He originally came to the dentist who was going to extract it because of the large facial swelling. After an incision and drainage and a couple weeks of Ca(OH)2 i finished the case. Looking forward to recalling this in a year!

Well summer is almost over so it’s time to get back to posting cases!

This was a complex prosthetic case referred by a prosthodontist who wanted retreatment of teeth numbers 7-10 with post spaces. Tooth 8 already had a large poorly placed post so I was concerned about a vertical root fracture due to that large lateral lesion but turns out it was a lateral canal!

Not very often you see a middle distal canal. It wasn’t a single ovoid canal, but rather a distinct canal that was formed when I attempted to instrument the isthmus between the distal canals. There was a middle mesial canal as well, but I wasn’t able to instrument beyond the coronal 6mm of the canal. Should heal just fine!

This tooth was referred for completion of a previously initiated root canal. The previous dentist had noted calcified canals and inability to get to length.

You’ll see in the post-op image all canals were able to gain patency with a middle mesial canal! There was a significant ledge in the distal canal that was able to be bypassed as well.

Adequate irrigation at length may be all you need to make sure all debris is removed from within an isthmus! Look at that obturation as the mesial canals merge!

Interesting retreat of a maxillary second molar.

Only two canals were obturated in this case with the distal-buccal not being instrumented and cleaned sufficiently leading to failure... the post-op shows the obturation of a single, large ovoid buccal canal!
