⚠️ Incident Prevention Starts with Preparation ⚠️

A recent well control incident highlighted how severe weather conditions can quickly escalate operational risks when critical subsea control systems are compromised.

During operations, damage to MUX communication cables and the hotline hose resulted in the loss of the primary BOP control system, leaving only limited secondary control options available. While the well was safely shut in and the LMRP recovered for repairs, the extended downtime allowed the well to completely gas out, creating significant re-entry and well kill challenges.

Key lessons for the industry:

✅ Ensure storm loops are correctly configured with adequate slack to accommodate vessel movement during adverse weather conditions.

✅ Regularly inspect and verify umbilical arrangements, especially when operating in environments with elevated metocean risks.

✅ Maintain fully functional secondary BOP control systems and validate emergency response capabilities.

✅ Develop contingency plans that consider the unique risks associated with PMCD operations, including reservoir fluid migration during extended downtime.

✅ Consider earlier deployment of mechanical barriers when operational conditions deteriorate or loss mitigation efforts prove ineffective.

Last Friday, Subsea Solutions was proud to support the future of the oil & gas industry by sponsoring both a shooting team and a station setup at the API Houston Fundraiser benefiting the API Houston Scholarship Endowments. Today, we pause to remember the lives lost in the Deepwater Horizon tragedy 16 years ago. Eleven workers never made it home, and the impact of that day continues to shape our industry and our responsibility within it.

During an inspection on a rig in the GOA, external leakage was noted on an annular that was refurbished by the OEM.

Subsea Solutions inspection process located the issue with an elastomer incorrectly installed in the wrong direction.
