MAI is a DPH funded program that offers a reconnect or connect to care services using the referral process. Providers should submit referrals along with updated releases and consents. MAI then can seek out a client who has missed 2 or more appointments with Case Management and or has outdated lab work over 6 months or missed Ambulatory Care appointments.
