Esthetic Endodontics. This tooth had trauma 20 or more years ago. Asymptomatic, but had been discoloring for years. The main concern of course was the apical radiolucency, but it is so much easier to do internal bleaching at the same time, and possibly not even need a crown or veneer. One round of sodium perborate and we got this result! Often times another round or two is needed. This was one happy patient who said “my daughter said, daddy your tooth isn’t brown anymore!” This was a very happy patient!

We saw this case back for a 6 month recall this week. It’s always amazing how people can have such large lesions and not even know it! With such a large lesion a root fracture is always a possibility, but the patient wanted to try to save it, and so did we! Sure glad we did! The previous root canal treatment had some deficiencies. We obtained additional length in all canals, treated additional distal canal anatomy, and did a few weeks of calcium hydroxide. No fractures were noted internally. Pretty significant healing for 6 months on such a large lesion! Furcation bone almost completely restored with PDL and lamina dura. A little apical healing yet to go but it’s looking good! Happy and completely asymptomatic patient!
