🌟 Meet Cade! 🌟
We're excited to introduce Cade Horn, Family Nurse Practitioner and proud Baldwin County resident!
Cade is a Family Nurse Practitioner and a proud two-time graduate of the University of South Alabama, where he earned his Master’s degree in Family Nursing Practice. Before becoming a nurse practitioner, he spent five years as a registered nurse caring for patients in pediatrics, as well as pediatric hematology and oncology, in Mobile, Alabama.
When he's not in the clinic, Cade enjoys spending time with his wife, Emma, their beautiful daughter Annie, and their golden retriever, Ivy. Family is at the center of his life, and when he’s not caring for patients, you’ll likely find him spending time with loved ones, exercising, grilling, reading, enjoying the outdoors, or cheering on his favorite team, the Chicago Cubs. ⚾
📌Fun Fact: Cade is a huge fan of the Baldwin County food scene, and Jesse's in Magnolia Springs is his go-to favorite restaurant!
Most importantly, Cade is passionate about partnering with patients to improve their health and becoming a trusted part of the community he serves.

We had the best time at Career Day at Elberta Elementary! 💙
Charity, Dawn, and Dr. Tomaso loved getting to visit with the students and share a little about what we do each day at the clinic.
It’s always special getting to pour back into our community and maybe even inspire a few future healthcare workers along the way! 🩺✨

Y’all may have already seen her smiling face around the clinic, but we wanted to take a minute to properly introduce one of our own 💛
✨ Lacey Santa Cruz has been with us for a little while now here at Elberta Family Practice Clinic, and we are so thankful to have her as part of our team!
🩺 A little about her:
Lacey is a dedicated Family Nurse Practitioner and nurse educator with a strong foundation in both clinical practice and academia. She earned her Bachelor of Science in Nursing from the University of South Alabama and went on to complete both her Master of Science in Nursing and Doctor of Nursing Practice at Jacksonville State University. Her professional experience includes five years as a pediatric registered nurse before becoming a nurse practitioner. In addition to her clinical work, Lacey serves as a full-time nursing faculty member at Columbia Southern University. She is passionate about preparing future nurses through effective teaching strategies, meaningful mentorship, and the integration of real-world clinical experiences into the learning environment.
Lacey is a life-long resident of Baldwin County, along with her husband, Caleb Santa Cruz, who serves as the youth pastor for Summit Church. Together, they have a young son and two dogs. Ministry is a central part of her life, and she values opportunities to serve and connect with others through it. In her free time, she enjoys reading, exploring her interest in American history, and discovering new foods, especially when traveling with her husband. She also values outdoor leisure time, whether relaxing with her family, walking their dogs, or enjoying nature as a way to recharge.
Lacey looks forward to building lasting relationships and serving the community she has always called home.
