After more than a decade of visionary leadership, Care Synergy President and CEO Tim Bowen has announced his plans to retire on March 2, 2027.
Tim was Care Synergy's first employee and has helped build the organization into one of Colorado's leading nonprofit networks for hospice, palliative care, aging, PACE, and community-based healthcare services. His 40-year career in hospice care has left a lasting impact on countless patients, families, employees, and communities.
To ensure a seamless transition, Care Synergy has named Hospice Chief Operating Officer Tricia Ford as CEO Elect. Tricia brings more than 30 years of healthcare leadership experience and has spent nearly a decade helping guide Care Synergy's growth, innovation, and mission-driven work.

This week, colleagues from across Care Synergy and its Affiliates gathered to celebrate and honor Pat Mehnert as she begins her well-earned retirement after seven years of dedicated service.
During her time with Care Synergy, Pat served first as Director of Corporate Compliance and Risk Management and later as Vice President of Quality, Compliance, and Education. Her leadership, expertise, and commitment have helped shape our organization in countless ways.
As many team members shared stories and reflections, Care Synergy President and CEO Tim Bowen noted that Pat truly embodied Care Synergy Mindset Principle #5: Assume Positive Intent. Pat consistently approached her work and her colleagues with kindness, trust, fairness, and a genuine belief in the goodness of others.
The celebration was filled with laughter, gratitude, and heartfelt memories—a fitting sendoff for a beloved colleague whose impact will be felt for years to come.

Dr. Hernandez brings extensive healthcare leadership experience, deep clinical nursing expertise, and a passion for innovation, education, and workforce development. Throughout her career, she has led cross-functional teams, developed engaging learning programs, and implemented data-driven solutions designed to improve clinical excellence and patient outcomes.
Her experience spans critical care, emergency medicine, neurology, home care, and healthcare education, along with expertise in instructional design, learning management systems, leadership development, and emerging healthcare technologies.

Care Synergy is pleased to welcome Vennita Jenkins as PACE President.
Vennita brings more than 20 years of leadership experience in senior services, healthcare operations, affordable housing, and nonprofit management. Throughout her career, she has built high-performing teams, strengthened community partnerships, and championed innovative, participant-centered care for older adults.
Her extensive experience includes leadership roles with Senior Housing Options and InnovAge Greater Colorado PACE, where she successfully led one of the nation’s largest standalone PACE centers serving more than 730 participants.
“Vennita’s deep experience in senior care, operational leadership, and community engagement makes her an exceptional addition to our organization,” said Care Synergy CEO Tim Bowen. “Her passion for serving older adults and commitment to high-quality, participant-centered care align perfectly with the mission and values of Care Synergy.”

Her extensive background includes leadership roles at The Elizabeth Hospice, The Denver Hospice, and previously within Care Synergy itself — making this an exciting return to the organization.
Throughout her career, Kerri has demonstrated a deep commitment to quality patient care, regulatory excellence, staff education, and continuous improvement across hospice and palliative care services.

Our Nursing Assistant Training Program is setting students up for success in a big way. With comprehensive training that includes classroom learning, hands-on simulation, and real clinical experience, our students are more than prepared—they’re excelling.
The results speak for themselves:
• 100% pass rate on the written CNA exam (compared to the national average of 70–80%)
• 82% first-time pass rate on the skills exam (well above the national average of 63–76%)

A new Medicare opportunity could help more older adults stay safe and independent at home—and Care Synergy and the CAPABLE National Center encourage healthcare organizations to apply.
The Centers for Medicare & Medicaid Services has launched the LEAD Model (Long-Term Enhanced ACO Design). Embedded within this initiative is RISE (Resilience and Independence in a Safe Environment), a falls-prevention program modeled after the evidence-based CAPABLE approach.
RISE is designed to help older adults:
• Reduce falls
• Maintain independence at home
• Perform daily activities like bathing, cooking, and dressing more easily
Healthcare organizations forming or joining an Accountable Care Organization (ACO) are encouraged to apply. By participating, organizations can help bring proven falls-prevention strategies into mainstream Medicare and improve quality of life for older adults.
