We do an assessment to determine needs and risk factors related to the individuals current condition by establishing a care plan with a Case manager. Care Plan includes:




Proper Id (license, birth certificate(s), social security card)

Assessment for getting food stamps

Client income/job status evaluation

Metro Health care( if needed and client is willing)

Help locating counseling groups
