example of how organizations can work together for common goals, both formally and informally, to drive innovation and partnership in solving challenging health care problems. CHP’s role is to facilitate coordination among health care providers and to identify ways we can work together to address community health issues. We are a membership-based organization whose sole purpose is to provide a forum for open dialogue based on an underlying value of doing what is best for our community and help to ensure that health care decisions are made locally. CHP members know each other well, are open to new ways of working
together, and are not afraid to take chances. Our coalition is built on a

CATCH
(Coordinated Access to Community Health)
Thanks to Medicaid expansion, the number of uninsured residents has been cut in half and has stabilized at around 6%. Even so, there are still local residents who have no insurance or who cannot afford the co-pay and high deductibles required by their plans. CATCH’s work is not done. The CATCH system of safety net clinics and other providers continues to meet monthly to share information and resources for assuring access to health care for our community’s most vulnerable residents. COMMUNITY DETOX
CHP was invited to co-chair the Community Detox Coordinating Council
last year. The group, made up of medical and behavioral health providers, emergency responders, El Paso County officials, and others meets quarterly to provide input into the County’s operation of our community’s detox operation. Detox served more than 4,000 people last year. CHP participates in all four detox workgroups: Coordinating Council (community input, overall governance);
Finance Committee (sustainable funding and budgeting);
Long Term Planning (designing a sustainable, comprehensive, tiered detox system for the future);
and Medical and Behavioral Clinical Oversight (establishing
protocols and medical standards of treatment). HEALTH INFORMATION EXCHANGE (HIE)
The community HIE system, coordinated by CHP, has moved from
demonstration to implementation. Working closely with CORHIO
(Colorado Regional Health Information Organization) and local providers, we have successfully exchanged critical patient information between ambulatory and acute providers—the first time in Colorado! Our project is leading the way for other regions of the state. Simultaneously, CHP is hosting the community’s data warehouse of both claims and clinical data for Medicaid, providing a rich source for the management of care at the population level. And, the project is
under budget and on time! HOMELESSNESS
CHP works with community social services and medical providers to address issues of homelessness. CHP participates in the Mayor’s Homelessness Trustees and the Continuum of Care, funds care for homeless individuals discharging from the hospital through Ascending to Health Respite Care, coordinates care for super-utilizers of 911 and emergency department services with the CS Fire Department CARES program of community paramedicine, and provides Bridges Out of Poverty training around health disparities related to low income individuals for providers. COMMUNITY CARE OF CENTRAL COLORADO
(RCCO Region 7)
Over 165,000 Medicaid clients in a four-county region are served by CHP’s Community Care program. Preserving our strong commitment to the community, since the beginning of the RCCO contract in 2011, nearly $18.2 million has been invested in care in the community through delegated care coordination, pay for performance programs, and RCCO services provided by CHP member organizations. Additionally, $6.4 million have been invested in innovative pilots and programs. Overall, nearly 63% of our contract revenue has been passed through to the community.
