This is why perimenopause rarely shows up as one symptom. Estrogen has receptors in tissues throughout the body, so when it starts fluctuating, the effects appear in several places at once - and often none of them look hormonal on the surface.

Aching joints. Skin that changed. Sleep that got lighter. Blood sugar that behaves differently than it used to. A mood that swings further than it did. Each one tends to get treated as its own separate issue with its own separate explanation.

Seen together, they usually make more sense than they do apart.

The encouraging part is that the same is true in reverse. Support the systems underneath - steady blood sugar, protein, sleep, movement, inflammation - and several of those symptoms tend to ease together, because they were never separate to begin with.

The aches that suddenly show up in your 40s aren't always simply "wear and tear."

Joint and muscle pain becomes more common during perimenopause, and changing estrogen may be one reason why.

Estrogen receptors are found throughout muscles, tendons, cartilage, and other connective tissues. As estrogen fluctuates and eventually declines, it can affect collagen production, tissue health, pain sensitivity, and inflammatory signaling.

But not every new ache is hormonal.

Thyroid dysfunction, nutrient deficiencies, poor sleep, chronic inflammation, autoimmune conditions, metabolic or gut health, and previous injuries can all contribute - and several can overlap.

Supporting the basics still matters: adequate protein, resistance training, quality sleep, and the nutrients needed for healthy muscles and connective tissue.

But if your body starts aching in ways it didn't before, don't automatically write it off as age.

The better question is: what changed?

Bone density was the last thing on my mind in my 30s. Then a test at a conference showed my bones looked like they belonged to a woman in her 90s. I was told, more or less, that I had terrible bones.

I've changed a few things in my life. And had a complete turnaround since then. And now I notice bone-related markers coming up high on hormone testing in many of my perimenopausal and postmenopausal clients.

Bone loss speeds up most about a year before your final period, often while cycles still look normal, with no symptoms and nothing on standard bloodwork. For some women, perimenopause starts in the late 30s.

Women's immune systems work differently from men's - and hormones are part of the reason.

Women generally produce stronger immune responses and often more antibodies. Estrogen influences many immune cells, helping regulate how strongly the immune system responds.

That stronger response has advantages, but it may also contribute to why autoimmune conditions disproportionately affect women.

And because estrogen and other hormones change throughout the menstrual cycle, perimenopause, and menopause, immune activity isn't static either.

This is why I don't think of immune health as simply "boosting" immunity. The goal is healthy regulation.

Protein provides building blocks for immune cells, stable blood sugar helps reduce metabolic stress, and quality sleep supports immune regulation and recovery.

If you're frequently getting sick, taking longer to recover, or noticing symptoms changing alongside your hormones, there may be more to look at than simply "low immunity."
