Community Care Program Eligibility: Individuals aged 60 years or older, residing in Illinois, who are U.S. citizens or eligible non-citizens falling within specified categories, possessing non-exempt assets totaling $17,500 or less (excluding exempt assets like home, car, and personal furnishings), and demonstrating an assessed need for long-term care based on the Determination of Need (DON) assessment to be at risk for nursing facility placement.
