Most countries adopt the definition of quality, safety, and efficacy comparability to the reference product, but gaps remain in clinical study waivers, interchangeability guidance, and pharmacovigilance.

Recent guidelines in advanced economies permit abbreviated pathways based on robust analytical evidence, while many emerging economies require additional clinical and immunogenicity data. Regulatory inconsistencies may hinder cross-country data sharing and biosimilar access.

Enrollment in Institutional Special Needs Plans (I-SNPs) was associated with significant reductions in hospitalizations, intensive care unit admissions, and mechanical ventilation during the last month of life for nursing home residents with dementia, without increasing hospice use.

The effect was larger for UnitedHealthcare (UHC) I-SNPs compared to non-UHC I-SNPs, and UHC plans demonstrated spillover benefits to nonenrolled residents in participating facilities. Plan maturity and volume appear important for maximizing impact on end-of-life care quality.
