Big news! We’ve officially been named a 2026 Bronze National Quality Award recipient by AHCA/NCAL! This honor recognizes our team’s commitment to delivering high-quality, compassionate care. We’re proud, and we’re just getting started!

🎣 One of our favorite days of the year — the annual Three Rivers Care fishing trip! There’s nothing better than getting outside, enjoying the fresh air, casting a line, and spending quality time together.

While the fishing is always fun, the best part is the memories made and the time spent with one another. We are so thankful for our incredible Life Enrichment team, our amazing volunteers, and most importantly, our residents who are family.

Days like these remind us that it’s the simple moments and meaningful connections that matter most. ❤️🐟☀️

Three Rivers Care is having their Summer Festival on June 26th!! Lots of great local food vendors, and awesome things to see, so make sure to stop in at 1305 Alexander St, in Centralia! It will be from Noon - 4 PM. More details below!

Join us for an afternoon full of fun, community, and amazing local vendors including Traveling Tales WA with their mobile bookstore, The PNW Scoop, Lewis County Canine Co., Live 95 and the giveaway Jeep! ✨🐶📕

We’ll also have face painting, a bounce house, treats, some classic cars, activities, and so much more for the whole family to enjoy!

Come out, celebrate summer with us, and enjoy a fun day with our community. We can’t wait to see everyone there! ☀️🍦

We had the best time dropping off appreciation goodies to our amazing EMS crews for EMS Appreciation Week!

Skilled Nursing Week was filled with spirit days, fun outfits, delicious food and goodies, and celebrating!

Celebrating the incredible mothers who fill Three Rivers with love, compassion, and care. This week’s Mother’s Day Tea was a beautiful time shared with our residents, staff, and families as we honored the amazing moms who live here and the devoted mothers who work here every day. Thank you for all the love you give so freely, today and always. Happy Mother’s Day!

We had such a fun week celebrating our amazing nurses for Nurses Week! We also had a very successful Skills Fair. A huge thank you to Assured Hospice for joining us this week to help re-educate our staff on palliative, end-of-life, and comfort care services.

We are so lucky to work alongside so many incredible nurses. They are truly the heartbeat of our facility and our industry. Thank you to all of the amazing nurses who give so much of themselves every single day. We appreciate you 365 days a year — but we’ll gladly spoil you a little extra this week for fun! 💙

We’ve been waiting patiently for Spring so we could finally get our patio ready! We were able to officially break it in today with a Staff Appreciation BBQ and we can already tell it’ll be a great hit for residents, families, and staff! Residents have already been out sunbathing!
Activities has so many plans for planter boxes and gardening, too. We’re ready for a fun sunny season! 🌺☀️

We had the BEST time at our Easter Egg Hunt this weekend! 🐰🥚🌷
What an amazing turnout, and we were so excited to have a special visit from the Easter Bunny himself! The kiddos had an absolute blast hunting for eggs, and our residents loved seeing all the smiling faces and being part of such a joyful day.
