🤝 JFS at Home Strengthens Partnership with Baptist Health

JFS at Home is proud to strengthen our partnership with Baptist Health through our “Hospital-to-Home” initiative!

Leaders from JFS and JFS at Home met over the summer with Baptist Health nurses, social workers, and discharge planners to discuss how we can work together to support safe, seamless transitions from hospital to home.

Our team shared information about JFS Senior Services and JFS at Home’s private duty homecare programs, with a special focus on our Hospital-to-Home program — designed to provide immediate support after discharge and connect patients and families with additional services and resources when they need them most.

This collaboration helps provide peace of mind for families during challenging transitions while ensuring patients receive compassionate, coordinated care at home.

We are grateful for the warm reception from the Baptist Health team and look forward to growing this important partnership together.

Welcoming a New Chapter in Leadership

JFS at Home is proud to announce the appointment of longtime community leader and founding Board member Ivan Gefen as Chair of our Board of Directors, succeeding Ed Levine.

Ivan has been part of JFS at Home since its inception in 2016 and brings decades of nonprofit leadership, community involvement and financial expertise to his new role.

“Having been involved with the organization from the beginning, I have seen firsthand the extraordinary care, compassion and peace of mind it provides to older adults and their families,” Ivan said. “I look forward to building upon that strong foundation and expanding our impact throughout the community.”
