When a ransomware attack exposes more than just files.

In April 2021, OSF Healthcare System discovered that files on its systems had been infected with the Nephilim ransomware variant.

An investigation later determined that threat actors had stolen the protected health information of 53,907 individuals.

The exposed information included:
• Driver’s license numbers
• Diagnosis and treatment information
• Prescription information
• Medical record numbers
• Health insurance information
• Financial account information

In July 2026, the U.S. Department of Health and Human Services (HHS) Office for Civil Rights announced a settlement with OSF Healthcare System for $552,250.

But the financial penalty wasn't the only consequence.

As part of the corrective action plan, OSF must conduct a thorough risk analysis, develop a risk management plan, and take additional steps to protect electronic protected health information (ePHI). HHS will monitor the corrective action plan for two years.

The lesson for healthcare organizations?
