To my dental colleagues,
I'm writing to let you know about a service that I've been providing to dentists and specialists for a number of years--implant rescue treatment. I've devoted a portion of my clinical practice to rescuing implant-compromised cases related to screw and abutment fractures. I credit my high success rate to many years of experience, dental microscope use in all of my operatories, and a strict treatment protocol. Not all implant cases can be rescued. The following implant was placed 15 years ago. The patient was referred by a dentist for implant rescue of a loose implant crown.
The following implant rescue procedure was performed:
1. External Splinting - to stabilize the implant supported crown during the
drilling phase
2. Pre-soaking of the implant abutment screw head to loosen the screw
3. Removal of the Gold-Tite screw with a lever driver
4. Microscopic examination of the implant fixture - determined that the
implant fixture was fractured at the coronal portion
5. Removed the fractured component with cotton pliers
6. Placed cotton and PVS plug in the top portion of the implant fixture
Recommendation - Refer to implant surgeon and have the broken implant fixture
replaced with a new implant fixture. Recommended an internal conical connection design
- i.e. Straumann RC BL
Implant Rescue - A Team Approach

Implant Rescue - This case involved precise core drilling (.25mm diameter). After removing the broken implant screw, I referred the patient back to the dentist to place an additional implant just behind it. Two new crowns will be made and joined together for strength. Happy implants, happy patient!!

Patient was referred to me for implant rescue. The case included:
1. Initial radiograph
2. Pre-op photos
3. Laser treatment
4. Square end driver
5. Conical connection surface identification
6. Implant brand identification
7. Treatment plan
There appears to be a miss match between implant abutment/fixture interfaces. The abutment that was presented with the crown did not match the true hex configuration of the fixture located in the patient’s mouth. A Branemark healing abutment was placed and a new radiograph was taken to verify fit.

A Key West dentist referred his patient to me for implant rescue. The implant fixtures were identified as Straumann BL RC from the X-rays taken. The implant abutments and screws were removed by conventional and drill out slotted methods. New healing abutments were placed and the patient was referred back to his dentist for continued care. A Leica MS5 microscope was used to treat this case.
