Implant Mechanical Rescue
Over the years, performing Implant Mechanical Rescue on challenging implant cases has broadened my understanding of not only what can go wrong, but more importantly, why it happens.
The fundamental steps of removing a loose implant screw, stripped screw, seized abutment, fractured screw, or fractured abutment provide valuable information about the mechanical forces that affected the implant restoration.
But the bigger question is:
Why did it happen in the first place?
Implant abutment designs vary among manufacturers. Implant treatment design varies with each surgeon and restorative dentist. Occlusion, component geometry, implant positioning, loading, and the relationship between all of these factors can have a profound effect on long-term mechanical stability.
One principle that consistently influences the long-term success of an implant restoration is the physics of the design.
Every fractured component tells a story.
Through Implant Mechanical Rescue, we have the opportunity to learn from those failures—not simply to remove the broken component, but to understand the mechanical forces and design principles that contributed to the failure.
That understanding can help us make better decisions when planning and restoring the next implant case.

To my dental colleagues,
I'm writing to let you know about a service that I've been providing to dentists and specialists for a number of years--implant rescue treatment. I've devoted a portion of my clinical practice to rescuing implant-compromised cases related to screw and abutment fractures. I credit my high success rate to many years of experience, dental microscope use in all of my operatories, and a strict treatment protocol. Not all implant cases can be rescued. The following implant was placed 15 years ago. The patient was referred by a dentist for implant rescue of a loose implant crown.
The following implant rescue procedure was performed:
1. External Splinting - to stabilize the implant supported crown during the
drilling phase
2. Pre-soaking of the implant abutment screw head to loosen the screw
3. Removal of the Gold-Tite screw with a lever driver
4. Microscopic examination of the implant fixture - determined that the
implant fixture was fractured at the coronal portion
5. Removed the fractured component with cotton pliers
6. Placed cotton and PVS plug in the top portion of the implant fixture
Recommendation - Refer to implant surgeon and have the broken implant fixture
replaced with a new implant fixture. Recommended an internal conical connection design
- i.e. Straumann RC BL
Implant Rescue - A Team Approach

Implant Rescue - This case involved precise core drilling (.25mm diameter). After removing the broken implant screw, I referred the patient back to the dentist to place an additional implant just behind it. Two new crowns will be made and joined together for strength. Happy implants, happy patient!!
