It’s National Diabetes Week, and we are proud to highlight the work happening across LifeLong to support patients living with diabetes.

Managing diabetes is about more than numbers. It is about meeting patients where they are and supporting them every step of the way.

At LifeLong, diabetes care is a core quality measure, and we are proud to be making progress. Currently, 77.5% of patients in this group have received the recommended screening.

We combine early detection, consistent follow-up, and innovative tools to improve outcomes. Through our partnership with Gojji patients can use simple, cellular-enabled glucometers that automatically share readings with their care team. No apps. No barriers. Just real-time data that helps providers intervene earlier and adjust care when it matters most. To support re-engagement, we are also offering incentives, including gift cards and Gojji swag, to patients who consistently share their blood glucose readings and stay connected to their care.

But technology is only part of the solution. Our providers and quality improvement team work closely with patients to support long-term health and ensure care is proactive, coordinated, and responsive.

This is what it looks like to meet patients where they are, to show up consistently, and to deliver care that is proactive, connected, and rooted in trust.

This spring, Climate Health Promoters (CHP) Cohort 5 brought together 18 youth and young adults ages 15–22 to explore the connection between climate, food, and community health.

About half of the participants are LifeLong Medical Care clients, highlighting the program’s deep connection to the communities we serve.

From volunteering on Earth Day to exploring Vasco Caves Regional Preserve, participants gained hands-on experience while building confidence, connection, and leadership skills.

A highlight of this cohort was the return of past CHP graduates as co-facilitators, creating a powerful peer mentorship experience and showing the lasting impact of the program.

“At first, many youth were quiet and reserved. By graduation, almost all chose to present their projects live, stepping outside their comfort zones and sharing their work with pride,” said Nanci Andrade, Wellness Center Program Coordinator. “Watching their confidence and leadership grow was one of the most rewarding parts of this cohort.”

By the end of the program, participants were more engaged, sharing ideas, asking questions, and building connections within their communities. Several expressed interest in returning as future co-facilitators, demonstrating the strength and momentum of the CHP community.

On May 16, we celebrated 18 graduates at the Nevin Community Center in Richmond, recognizing months of growth, resilience, and dedication.

We are proud of these young leaders who are already creating positive change for themselves and their communities.

Our communities are strongest when access to care grows alongside the next generation of providers. We were proud to have our Chief Dental Officer, Dr. Miriam Parker, in attendance at the UCSF School of Dentistry graduation, celebrating graduates who are committed to advancing oral health care in the communities that need it most.

It was especially meaningful to celebrate alongside leaders and advocates such as UCSF Board of Regents member Janet Reilly, who continues to champion expanded access to care for underserved communities across the Bay Area, as well as students who have already made a difference through hands on service.

Two of these graduates rotated through LifeLong’s dental externship at our Berkeley health center during their senior year, helping expand access to care for our patients while gaining valuable real-world experience.

Through partnerships like ours with UCSF, we are helping prepare future providers while strengthening care for our community today.

The first-ever LifeLong Medical Care Oral Health Award has been presented to Henry Pineda Pere, one of the scores of Howard University dental students who have trained in LifeLong’s dental student externship program.

Chief Dental Officer Dr. Miriam Parker described Pineda Pere as providing “exceptional dental care to our patient population” during his four-week externship at LifeLong this year.

“Aside from providing one of the highest productivity numbers, he was punctual and professional,” Dr. Parker said. “Henry’s unwavering spirit with our clinical staff and dentists did not go unnoticed.”

Pineda Pere was one of sixteen 2025-2026 senior dental students at Howard University College of Dentistry selected by HUCD faculty to participate in the externship. Dr. Parker said that the vote by LifeLong’s dental preceptors and staff was overwhelmingly in Pineda Pere’s favor.

At a graduation ceremony held at the University, Pineda Pere received a personalized champagne bucket and a $1,000 check.
