Medical Release Form This is the Online Medical Release Form for Christ in Youth.

CIY is coming up July 26-31! The total cost for RUSH YOUTH is $150. Registration money ($50) is due by April 29, at RUSH! We are asking the last of the youth portion $100 be paid by May 20, our last regular scheduled RUSH. Summer Kick Off Party!
