Chock Indikationer.
Shock is a life-threatening condition of circulatory failure most commonly presenting with hypotension. Initially reversible, the effects can rapidly become irreversible, resulting in multi-organ failure. This type of shock can cause many organs to stop working as the shock reaches a "point of no return,” meaning the shock state becomes irreversible and the patient's death is inevitable due to insufficient or improper pumping of blood.
Hypovolemic shock is a condition in which severe blood or fluid loss makes the heart unable to pump enough blood to the body. (This is a "pre-load" problem for the heart: it can only pump what is delivered to it.) Hypovolemia is usually caused by hemorrhage (internal or external), resulting in reduced blood volume; or by extreme fluid losses seen in severe burns.
Although in the field most cases of hypovolemic shock are acute, due to trauma and hemorrhage, or burns, other causes of fluid loss are:
● GI losses from excessive vomiting or diarrhea,
● renal losses in kidney disease,
● ascites in liver disease, or
● skin losses from excessive exercise in dry climates.

The decision scheme below outlines criteria used for referral to a trauma center.
When choosing an appropriate facility, keep in mind the capabilities of the hospitals in your local and distant surrounding area. Remember that some hospitals can handle dozens of patients with enough warning, whereas others can become overwhelmed after receiving a few critical patients. In the event a hospital becomes overrun with a large number of patients, most facilities have a hospital surge capacity system in place to assist with the overflow. This may include tents or mobile units. Keep in mind, when appropriate to transport patient directly to specialty centers such as burn units or pediatric facilities that are more capable of handling particular patient situations.
Remember to communicate with the hospitals in your area. Early notification of an MCI to all receiving hospitals will give them time to prepare for an influx of patients, so they do not become overwhelmed themselves.
