🇬🇧Maxillary reconstruction with autologous bone using Khoury technique and insertion of 8 maxillary implants.

🇷🇴Reconstrucție maxilară cu os autolog prin tehnica Khoury și inserarea a 8 implanturi maxilare.

🇬🇧Patient wearing a prosthesis in the lower arch with terminal dentition in the upper arch, comes for a complete rehabilitation of the teeth.
We decide to insert 6 implants in the jaw and make an external sinus lift in quadrant II to avoid inserting an inclined implant. Another 6 implants are placed on the mandible for a better antero-posterior distribution.
Two prosthetic are performed. At the jaw, because there is no vertical bone resorption, we perform a FP1 type work to model the soft tissue and obtain a special aesthetic of the final work.

🇷🇴Pacient purtător de proteză la arcada inferioară și cu o dentiție terminală la arcada superioară, se prezintă pentru o reabilitare completă a danturii.
Decidem să inserăm 6 implanturi la maxilar și să facem un sinus lift extern în cadranul II pentru a evita inserarea unui implant înclinat. La mandibulă se pun alte 6 implanturi pentru o mai bună distribuție antero-posterioară.
Se realizează două lucrări protetice. La maxilar, din cauză că nu există resorbție osoasă verticală, realizăm o lucrare tip FP1 pentru a modela țesutul moale și a obține o estetică deosebită a lucrării finale.

🇬🇧We removed a large cyst in the jaw, after which we decided to reconstruct the defect with autologous bone, using the Khoury technique.
The two bone blades were fixed to restore the morphology, and then it was made a palatal pedicle from the connective tissue.

🇷🇴Am îndepărtat un chist mare la nivelul maxilarului, după care am decis să reconstruim defectul cu os autolog, prin tehnica Khoury.
S-au fixat cele două lamele osoase pentru a reface morfologia, iar mai apoi am realizat un pedicul palatinal din țesut conjunctiv. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀

🇬🇧 Periodontal compromised patient, we decided to rehabilitate the entire oral cavity. Because the clinical situation allowed us, we avoided inserting angled implants. An internal sinus lift was performed on the distal maxillary implants with Densah cutters.

🇷🇴 Pacient compromis din punct de vedere parodontal, s-a optat pentru reabilitarea întregii cavități bucale. Pentru că situația clinica ne-a permis, am evitat inserarea implanturilor angulate. Pe implanturile distale maxilare s-a realizat sinus lift intern cu frezele Densah.

🇺🇸 An interesting case that took almost a year to complete. The patient had advanced periodontal disease and no teeth could be preserved. Following the CBCT analysis, it was found that the volume of the maxillary bone did not allow the insertion of 6 implants.

In stage 1 we decided to increase the volume of the maxillary bone by performing a Khoury reconstruction in the frontal area and an external sinus lift in quadrant 2. The canines from the maxillar were kept to make a temporary fixed prothesis. At the level of the mandible, 4 implants were inserted and a temporary fixed prosthesis was applied.
In stage 2, 8 implants were inserted at the level of the jaw in the previously enlarged areas. The canines were cut off and Partial Extraction Therapy (PET) was performed to preserve the volume of the bone ridge and a temporary prothesis was performed on 4 of the implants inserted in the anterior area, fixed on the Multiunit. In front of the multiunits, pedicle grafts of connective tissue from the palace were sutured.

🇷🇴 Un caz interesant care a durat aproape un an ca să fie finalizat. Pacienta s-a prezentat cu boală parodontală în formă avansată și niciun dinte nu a putut fi păstrat. În urma analizei CBCT-ului, s-a constatat că volumul osului maxilar nu permitea inserarea a 6 implanturi.

În etapa 1 am decis să mărim volumul osului maxilar prin realizarea unei reconstrucții Khoury în zona frontală și un sinus lift extern în cadranul 2. S-au păstrat caninii de la maxilar pentru a realiza o proteză provizorie fixă. La nivelul mandibulei s-au inserat 4 implanturi pe care s-a realizat o proteză provizorie fixă.
În etapa 2 s-au inserat 8 implanturi la nivelul maxilarului în zonele augumentate anterior. S-au retezat caninii și s-a practicat Partial Extraction Therapy (PET) pentru a conserva volumul crestei osoase și s-a realizat o proteză provizorie pe 4 dintre implanturile inserate în zona anterioară, fixată pe Multiunit. În dreptul multiuniturilor s-au suturat grefe pediculate de țesut conjunctiv din palat.
