A chatbot that hallucinates 3 to 27 percent of the time is still confident every single time it answers.

That range comes from industry estimates cited in coverage of the Air Canada bereavement fare case, where a support bot invented a refund policy that never existed. The airline argued in court that the chatbot was a separate legal entity, not bound by company policy. The tribunal disagreed and made Air Canada pay for what its own bot said.

That is the part most teams miss when they plug in an AI layer. The model does not know when it is wrong. It says the invented policy with the same tone as the real one.

Guardrails are not a nice to have on top of an AI system. They are the difference between a tool that assists your team and one that quietly makes promises on your behalf.

95% of AI pilots never make it to production.

That is not a Stackup number. That is MIT.

A study released this year looked at 300 public AI deployments across enterprises. Only 5% delivered measurable financial impact. The rest stalled somewhere between the demo and the rollout.

The instinct is to blame the model. Wrong reasoning, hallucinations, not enough training data. But that is not what the research found.

The failures traced back to something simpler. Brittle workflows. Weak contextual learning. AI bolted onto a process that was never mapped out in the first place.
