3. I-fill out ang Personal Details

• Birthday, gender, civil status
• Contact number at email

4. I-fill out ang Travel Details
• Flight number at date of travel
• Airline at seat number (kung meron)
• Port of exit o entry

5. Health Declaration
• Sagutin ang travel history, sintomas, o health concerns.

6. Submit at i-save ang QR Code
• I-screenshot o i-store sa phone.
