𝗪𝗵𝗲𝗿𝗲 𝘁𝗼 𝗦𝘂𝗯𝗺𝗶𝘁: School Principal’s Office, Candaping National High School

𝗔𝗽𝗽𝗹𝗶𝗰𝗮𝘁𝗶𝗼𝗻 𝗥𝗲𝗾𝘂𝗶𝗿𝗲𝗺𝗲𝗻𝘁𝘀:

1. Application Letter signed by the parent or guardian
2. Learner’s Profile (SPS Form 3.2) with a 2x2 picture (See attachment at the comment section.)
3. PSA Birth Certificate or from the Local Civil Registrar
4. SF9 or Learner’s Progress Report Card
• Grades of 87 and above in Math, English, and Science
• No grade below 85 in other subjects (From Grade 6, 1st to 3rd Quarter)
5. Certificate of Good Moral Character
6. Certificates of Academic and Non-Academic Awards (Arrange from the latest)
7. Declaration of Medical Condition Form (SPS Form 3.0) (See attachment at the comment section.)

𝗜𝗺𝗽𝗼𝗿𝘁𝗮𝗻𝘁 𝗗𝗮𝘁𝗲𝘀 𝘁𝗼 𝗥𝗲𝗺𝗲𝗺𝗯𝗲𝗿:
March 16, 2026 - Qualified applicants will take the Mental Ability Test (30%) and the Communication Skills Test (30%).
March 17, 2026- Applicants will participate in Project Pitching (40%).
